Cakewalk or Utra-Unfair: What is the deal with the combat system?

I keep running into the same problem in this system's combat stuff... No matter what all fights end up either one of two things "Cakewalk" or "OMG UTRA-UNFAIR" and even when it's unfair, I've yet to seen a fight be lost in this system. Sure when it has it's moments it's really fun combat system. But there is never any "challenge" to the fights at all. And I don't think it's possible to add any unless you put in something unfair to the levels of "rocks fall" damage.

I don't get that... I just don't. And wanta see if anyone else here has found this same problem.

I think it really depends on what you throw at players and how teams are constructed.  From my meager GMing experience, I think smaller teams are easier to actually threaten in this system since there's less defense.  I feel solos are poor design by default unless they are multipart battles.  I also feel one of the better GM tools to shake things up is actually taboo in many games.
