The only problem with something like this is the calculation on how much vuln and Duelist has to be accounted for to make it fair. Without escalating non-random damage, vuln isn't as much of an outright problem yet it can still have use. Perhaps the d4 could be upped to d6.

What if the d4s (or d6s if you go with Xel's suggestion) were changed to lose life while the final hit still did damage? Actually, if all but the final hit were lose life, could it possibly regain some form of the escalation mechanic the original had?

For the escalating version it would feel rather weird. The static d4 version was actually a reference to Fist of the North Star in the way it was envisioned; Multiple pressure point strikes ending with an actual big hit at the end.

I rather like Cacophony of Death. Mechanically. The name could really be more musical sounding. Like some kind of Dirge or Funeral March or such.

So each die roll does not count as a separate attack, like Crescendo was.

Correct. If it was a multi-attack, it would use the term "deal X damage" more than once.

How about Death Knell? it has Death, a sound, and it's a saying.
Otherwise I like "Dance of Death" or "Dark Requiem"
I think "Dark Requiem" sounds coolest, Death Knell is nice because of how quick and easy it is to say, and Dance of Death is cool too but a little bit more of a mouthful.

Wow, Red Requiem has a really cool ring to it.
Still, Death Knell sounds the most "Lethal" as Dan put it - while many sound like an extravagant necromatic ritual, DK really evokes the feeling of a necromancer snuffing the life out of people best.

Dance Macabre is still my favorite. Second place (far far second) would be Dark Red Requiem.

That is a 120% opinion based statement... You do know that right? Cause to me Dance Macabre is quite cool to say and equal to Red Requiem in coolness factor... If anything I'd argue that Death Knell sounds lame and not that impressive when said.
