[-cost] Repeated Crack Fist [Standard Attack]
Deal 1d4 damage to target creature.
Deal 1d4 damage to target creature.
Deal 1d4 damage to target creature.
Deal 1d4 damage to target creature.
Deal 1d8 damage to target creature.

Without escalating non-random damage, vuln isn't as much of an outright problem yet it can still have use.  Perhaps the d4 could be upped to d6.
The only problem with something like this is the calculation on how much vuln and Duelist has to be accounted for to make it fair. Now, maybe if it was "lose hp"...

What if the d4s (or d6s if you go with Xel's suggestion) were changed to lose life while the final hit still did damage? Actually, if all but the final hit were lose life, could it possibly regain some form of the escalation mechanic the original had?

It very well could, in fact. Perhaps a Necro talent called "Cacophony of Death" or "Vile Requiem" - sound based names as a nod to the original

[-8] Cacophany of Death - Standard Attack
Target creature loses 1d4 hp

we *could* have the last one deal damage, but that feels so random...

I see the nickname of "Buck you Minions!" being given to this combat talent.

For the escalating version it would feel rather weird. The static d4 version was actually a reference to Fist of the North Star in the way it was envisioned; Multiple pressure point strikes ending with an actual big hit at the end.

I rather like Cacophony of Death. Mechanically. The name could really be more musical sounding. Like some kind of Dirge or Funeral March or such.

Danse Macabre? (Dance of Death) That's the first thing that came to mind for me.

Danse Macabre or Dance of Death sounds the best term to use.

Not for anyone that's played WoW! The pronunciation of the word is a bit silly as well (durr-ge).

As for other names, the Kingdom Hearts enemies got a few musical names that would fit the bill..

Black Ballade
Scarlet Tango
Raging Requiem

This might be the right place to ask this question. Is electrocution one single attack, or multiple attacks?

So each die roll does not count as a separate attack, like Crescendo was.

Correct. If it was a multi-attack, it would use the term "deal X damage" more than once.

How about Death Knell? it has Death, a sound, and it's a saying.
Otherwise I like "Dance of Death" or "Dark Requiem"

I think "Dark Requiem" sounds coolest, Death Knell is nice because of how quick and easy it is to say, and Dance of Death is cool too but a little bit more of a mouthful.

Wow, Red Requiem has a really cool ring to it.

Still, Death Knell sounds the most "Lethal" as Dan put it - while many sound like an extravagant necromatic ritual, DK really evokes the feeling of a necromancer snuffing the life out of people best.

Dance Macabre is still my favorite. Second place (far far second) would be Dark Red Requiem.
Edit: Yeah, Red is better than Dark here.

Perhaps Red Requiem should be a Warlock talent, because blood.
My issue with Dance Macabre is that, while stylish, it doesn't sound too great verbally

That is a 120% opinion based statement... You do know that right? Cause to me Dance Macabre is quite cool to say and equal to Red Requiem in coolness factor... If anything I'd argue that Death Knell sounds lame and not that impressive when said.
